Have your own picture put on to mats and coasters. The hardest part is selecting the one, favourite image; it can be a photograph, or perhaps a special drawing or painting that you have done; and then send it to Jenny Duff. Please remember when making your selection that a "landscape" picture will work best as a table mat.

To receive your Presentation Box containing 6 Table Mats (19.1x24.2cm), all with the same image, delivered to you within six to eight weeks, please either upload your picture or photograph below or send it, with a copy of this completed form, either as a jpeg on cd or as a print not bigger than A4 (21cm x 29.7cm). Please use a Royal Mail signed-for service if your picture is very precious to you and send to Jenny Duff, 4 West Cliff Mansions, Cliff Street, Ramsgate, Kent CT11 9HY.

The total cost for this Special Personalised Service is £65.00 plus £6.95 towards postage and packing. Please pay by cheque with this order form, or telephone to order by credit card, if you prefer.
